Counselling Guides for Post-Discharge Care ~ Smart Discharges

Objective(s): Smart Discharges is a digital health program that uses individual-level risk prediction and intervention to increase effective health seeking behavior, improve health outcomes, and reduce child mortality during the post-discharge period. Health workers aim to mitigate risk by educating caregivers on post-discharge care and by scheduling follow-up visits for at-risk children in their communities.
Data Description: This dataset includes the following materials for caregiver counselling: 1) Caregiver counselling guide for Health Workers use; 2) Post-hospital discharge counselling cards for Health Workers use 3) Mother's counselling card (Truncated version) for Health Workers use 4) Post-discharge care counselling cards for community health workers use. Materials were originally developed by WALIMU, in partnership with the University of British Columbia, and were adapted by the Uganda Ministry of Health.
All materials are provided in the English language.
Limitations: These materials were designed for the Ugandan context and may not be generalizable to other settings.
