Online questionnaire for a University of Bamenda study on burnout among private primary school teachers in Mezam Division, Cameroon # Teacher Burnout Questionnaire Online data-collection instrument for the study: > **Social Challenges as Predictors of Burnout among Private Primary School Teachers > in Mezam Division, Cameroon** > University of Bamenda · Faculty of Education · Department of Counselling Psychology > Submitted in partial fulfilment of a Master's degree in Clinical Counselling. ## What is here | File | Purpose | |---|---| | `index.html` | The questionnaire itself — a single self-contained page, no build step, no dependencies. | | `Code.gs` | Google Apps Script that receives responses and appends them to a Google Sheet. | | `SETUP.md` | One-time setup: create the Sheet, deploy the script, connect the two. | ## Instrument 26 items, faithful to the approved appendix: - **Section A** — Demographic information (6 items) - **Section B** — Workplace conflict and burnout (5 Likert items) - **Section C** — Administrative support and burnout (5 Likert items) - **Section D** — Family–work conflict and burnout (5 Likert items) - **Section E** — Financial stress and burnout (5 Likert items) Likert scale: SA = 4, A = 3, D = 2, SD = 1. Every response row carries both the label and the numeric code for each item. > Section E items 2 and 3 are identical in the approved instrument, and are > reproduced here as printed. ## How it behaves - All items in a section must be answered before the respondent can continue; unanswered items are highlighted. - Responses are written to the device before any network attempt, so a lost connection cannot lose data — queued responses are sent automatically the next time the page is opened online. - Duplicate submissions of the same response id are ignored server-side. - `#admin` opens a passcode-protected summary with CSV and JSON export.
