Both forms are modeled on the type of fieldwork I've done professionally (health facility auditing, disability needs assessment) and both have been validated against the actual XLSForm specification, not just formatted to look correct. ## Forms included ### 1. SRH Facility Service Audit Form Modeled on the kind of routine facility audit I conduct as a Data Auditor. Demonstrates: - **Skip logic (`relevant`)** — follow-up questions on discrepancy details and escalation only appear when relevant (e.g., escalation notes only show if a finding is flagged high-risk) - **Validation (`constraint`)** — discrepancies found cannot exceed records reviewed, enforced at the point of data entry rather than caught later in cleaning - **Nested conditional logic** — a two-level skip: discrepancy details depend on discrepancies being found; escalation notes depend on that finding being flagged high-risk - **Bilingual labels** — English and Kiswahili, with proper ISO language codes (`en`, `sw`) ### 2. Disability Needs Baseline Household Survey Modeled on the baseline survey work I did with B&M Consult / Light for the World Kenya.
