Built for HMI Hackathon # VeriCare **A Prenatal Health Passport for Women Who Move Between Clinics** VeriCare is an offline-first maternal health system that helps pregnant women carry their care records with them across clinics. It is designed for rural and low-resource settings where internet access is unreliable, paper records are fragile, and important health information is often lost between visits. ## The Problem In many rural communities, pregnant women do not receive care from just one clinic. They may visit different clinics throughout pregnancy, and each new clinic often starts from zero because records do not travel with the patient. That creates serious problems: - missed warning signs - duplicate tests - delayed care - preventable complications - poor continuity between prenatal visits Existing digital systems often do not fit the reality on the ground. They can require internet, expensive hardware, or workflows that are too complex for busy clinics. SMS reminders can also expose sensitive health information on shared phones. ## The Solution VeriCare keeps the paper workflow nurses already trust, but makes it portable. At each visit, a nurse uses a basic Android app to scan a few key fields from the standard paper form, such as: - expected delivery date - blood pressure - hemoglobin - visit number The app then generates a secure QR code on the woman’s health card. At any participating clinic, a nurse can scan that code and instantly see the most important information needed for safe follow-up care. ### What the nurse sees - gestational age band - last blood pressure and hemoglobin readings - completed and missed ANC visits - risk flags - what care is needed today No full medical record is exposed. Only the information needed for continuity of care is shared. ## How It Works ### 1. Registration At the first visit, the nurse records the standard pregnancy information and a safe contact preference for reminders: - can SMS be used? - should messages stay neutral? - o …
