PRIME-AMR: Preventive Risk-Based Infection Management to Reduce Antimicrobial Resistance

Antimicrobial resistance (AMR) is accelerating faster than the development of new antibiotics, increasingly limiting effective treatment options and disproportionately affecting regions with limited diagnostic and laboratory capacity. Existing AMR strategies are predominantly reactive, focusing on resistance after it has already emerged. PRIME-AMR (Preventive Risk-Based Infection Management) presents a complementary, preventive framework that enables early assessment of treatment failure and resistance fixation risk before antibiotic therapy begins. The approach is based on structural risk profiling of infection states using readily available clinical data, minimal laboratory markers, and regional surveillance context. PRIME-AMR supports risk-oriented clinical decision-making while remaining fully compatible with existing WHO initiatives, including GLASS and the One Health framework. It is designed to operate in both low-resource (clinical data + minimal labs) and advanced healthcare settings, without requiring regulatory changes or new pharmaceutical development. The framework aims to reduce unnecessary antibiotic use, improve first-line therapy success, slow the emergence of resistance, and enhance global surveillance through early risk signals. PRIME-AMR is intended for pilot implementation, evaluation, and integration into national and international AMR programs. Устойчивость к противомикробным препаратам (УПП) распространяется быстрее, чем разрабатываются новые антибиотики, что все больше ограничивает эффективные варианты лечения и непропорционально затрагивает регионы с ограниченными диагностическими и лабораторными возможностями. Существующие стратегии борьбы с УПП в основном носят реактивный характер, фокусируясь на резистентности после ее возникновения. PRIME-AMR (Preventive Risk-Based Infection Management) представляет собой дополнительную профилактическую систему, позволяющую проводить раннюю оценку риска неэффективности лечения и фиксации резистентности до начала антибиотикотерапии. Подход основан на структурном профилировании риска инфекционных состояний с использованием легкодоступных клинических данных, минимального количества лабораторных маркеров и регионального контекста эпидемиологического надзора. PRIME-AMR поддерживает принятие клинических решений, ориентированных на риск, оставаясь при этом полностью совместимым с существующими инициативами ВОЗ, включая GLASS и концепцию «Единое здоровье». Он разработан для работы как в условиях ограниченных ресурсов (клинические данные + минимальное количество лабораторий), так и в условиях развитой системы здравоохранения, не требуя изменений в законодательстве или разработки новых фармацевтических препаратов. Цель данной программы — сократить необоснованное использование антибиотиков, повысить эффективность терапии первой линии, замедлить развитие резистентности и усилить глобальный эпидемиологический надзор за счет раннего выявления факторов риска. Программа PRIME-AMR предназначена для пилотного внедрения, оценки и интеграции в национальные и международные программы по борьбе с устойчивостью к антибиотикам.
