Data used for the primary analysis of the PreFIT study (contact data only). PreFIT investigated prospectively if contacts (household or close contacts) of tuberculosis (TB) patients with high C-reactive protein (CRP), low hemoglobin (Hb) levels, and a positive Xpert Host Response (HR) cartridge result develop active TB within 6 to 12 months Identified index cases who agreed to participate referred their household or close contacts to also join the study. Contacts (aged 12 to 60 years and both HIV negative and positive) were tested for TB at enrollment, those negative were followed-up at 6 months and 12 months for developing TB. Blood samples were collected at baseline and 6 months for testing. During the study period, TB testing was done on contacts who met symptoms criteria. At 12 months, all contacts also had a chest x-ray. The study was conducted in Mozambique (Manhiça Health Research Center, Manhiça), South Africa (Stellenbosch University, Cape Town), and Uganda (Makerere University, Kampala). Amsterdam UMC/Amsterdam Institute for Global Health and Development coordinated the project. PreFIT enrolled 1291 index TB patients and 3031 close and household contacts; 2538 contacts completed follow-up. Based on the primary endpoint (bacteriologically confirmed pulmonary TB by positive sputum culture or positive sputum Xpert MTB/Rif or Ultra result excluding Ultra trace results in the absence of a positive culture or with a positive sputum Xpert Ultra trace result in the absence of a positive culture and a history of TB treatment in the preceding 24 months), 37 contacts were diagnosed with co-prevalent TB (between enrollment and day 14) and 66 contacts were diagnosed with incident TB (between day 15 and end of follow-up).
