Background: Prostate cancer (PCa) is an increasing public health concern in Morocco, with a substantial proportion of cases still diagnosed at an advanced stage. This study aimed to describe the epidemiological, clinical, and therapeutic features of PCa in the eastern region of Morocco. Methods: We conducted a descriptive cross-sectional study including 74 patients managed in the eastern region of Morocco between June 2022 and May 2023, using medical records and a standardized questionnaire. Results: The mean age at diagnosis was 72.5 ± 8.7 years. Most cases were diagnosed after symptom onset, with lower urinary tract symptoms in 79.7% and bone pain in 44.6%. Median PSA at diagnosis was 76.5 ng/mL, and 77% of patients had PSA ≥20 ng/mL. Histology was dominated by adenocarcinoma (100%), with a high proportion of high-grade tumors (Gleason score ≥8: 52.7%) and metastases present at diagnosis in 33.8% of cases. Treatment relied mainly on hormone therapy (82.4%), external beam radiotherapy (25.7%), and radical prostatectomy (8.1%). Conclusions: This series highlights a frequently advanced presentation of prostate cancer in eastern Morocco. Our findings support the urgent need to strengthen targeted early detection strategies, improve diagnostic pathways, and expand access to curative options and modern evidence-based therapeutic standards.
