Abstract / Description Overview Sub-Saharan Africa bears a disproportionate burden of global maternal and neonatal deaths, accounting for approximately 70% of maternal mortalities and over 43% of newborn deaths worldwide. The vast majority of these deaths result from treatable or preventable complications including postpartum hemorrhage (PPH), pre-eclampsia, maternal sepsis, obstructed labor, and birth asphyxia compounded by severe shortages of skilled healthcare personnel, geographic isolation, delayed emergency transit, and fragmented medical communications. This paper investigates how recent breakthroughs in mobile technologies, edge-based artificial intelligence (AI), low-bandwidth communication channels (SMS/USSD), and autonomous logistics (drones) can move beyond theoretical framework toward real-world deployment across African health systems. Using Thaddeus and Maine’s Three Delays Model as an analytical framework, the study evaluates six key technological interventions: Mobile Pregnancy-Monitoring & Clinical Decision Support Systems (CDSS) for task-shifting to Community Health Workers (CHWs). AI-Assisted Point-of-Care Ultrasound (POCUS) utilizing ultra-portable probes and real-time computer vision "blind sweeps" to detect high-risk obstetric conditions. Behavioral SMS/USSD Messaging to improve antenatal care (ANC) appointment adherence and maternal health literacy. Cloud-Based Digital Referral Networks to streamline patient handoffs and alert regional surgical teams prior to patient arrival. GPS-Enabled Emergency Transport Networks & Autonomous Unmanned Aerial Vehicles (UAVs) for rapid dispatch and critical blood delivery. Digital Postnatal Surveillance Tools for continuous home monitoring and early detection of neonatal hypothermia and sepsis. Primary Outcome The study delivers a field-ready Technology-Enabled Maternal-Health Pathway, detailing an end-to-end integrated care architecture that seamlessly bridges pregnant women, community health workers, village clinics, and regional tertiary hospitals. Additionally, the paper provides policy-makers with a strategic 4-phase implementation roadmap to guide digital infrastructure standards, regulatory approvals, offline-first system resilience, and sustainable national financing.
