Introduction and Objective: Type 1 Diabetes Mellitus (T1DM) and the associated morbidity and mortality is a growing public health concern in Tanzania, with an estimated 84% of patients being diagnosed at diabetes ketoacidosis. Early detection and effective management are critical in reducing complications, yet resource constraints hinder timely diagnosis and intervention. This exploratory design process aimed to explore user needs and inform the development of digital health solutions that could enhance early detection, improve retention to care and streamline data management for T1DM in resource-constrained settings. Methods: A User Centered Design (UCD) approach was utilized, involving a sequence of interviews and focus group discussions with patients, caregivers, healthcare providers and other stakeholders in Tanzania, Dar es Salaam region. This process sought to identify challenges in care and utility and desired functionalities of potential digital interventions for T1DM. Prototypes were developed, tested and iterated based on user insights Results: Key barriers identified included late diagnosis, poor linkage and retention in care, inadequate access to education resources and weak data management processes. Users highlighted the need for integrated digital solutions to support service delivery, clinical decision support, patient education and data management. The prototypes demonstrated high acceptability from the users, with feedback indicating potential for health worker adoption and increased patient engagement. Conclusion: This UCD identified key barriers and user needs relevant to the development of digital solutions for T1DM management. The findings directly support the need for an integrated digital tool to enhance early diagnosis, improve adherence and strengthen self management. Investing in this solution could address key gaps in care, streamline data management, and support both patients and healthcare providers in delivering effective T1DM care in resource-limited settings.
