This dataset contains individual-level survey responses from 2,289 postpartum women in rural districts of the Sidama region, Ethiopia. Data were collected from February 1 to March 30, 2025, using a community-based cross-sectional study design with multistage sampling. Variables include socio-demographic characteristics (maternal age, education, occupation, household wealth index), obstetric history (gravidity, parity, history of stillbirth, abortion, antenatal care utilization, place of delivery), neonatal factors (sex, perceived birth size, congenital anomalies, twin pregnancy), and community-level factors (maternal knowledge of neonatal danger signs, road accessibility to health facilities, decision-making autonomy, husband's attitude toward neonatal health care). The dataset was used to estimate neonatal mortality rates and identify factors associated with neonatal mortality. The neonatal mortality rate in this population was 47 deaths per 1,000 live births (95% CI: 39-57). This dataset is valuable for researchers, policymakers, and practitioners working to reduce neonatal mortality in resource-limited settings, particularly in rural Sub-Saharan Africa.
