This dataset contains de-identified survey responses from a community-based cross-sectional study of 420 young people aged 15–24 years in Kichinjaji Ward, Soroti City, Eastern Uganda, examining uptake of youth-friendly reproductive health services (YFRHS) and associated factors. Data were collected via structured interviewer-administered questionnaire (March–June 2024) and cover sociodemographic characteristics, sexual and relationship history, awareness of YFRHS, service utilization, and parental communication about sexual and reproductive health. All direct identifiers (GPS coordinates, submission metadata, form numbers) have been removed prior to deposit. The accompanying Stata do-file (YFRHS_analysis.do) reproduces the descriptive, bivariable, and multivariable analyses reported in the associated manuscript.
