Real-time community disease intelligence for Ghana Health Service # Aduru Sentinel ## Real-Time Community Disease Intelligence for Ghana Health Service Aduru Sentinel is an early-warning disease surveillance platform that detects outbreak signals and antimicrobial resistance (AMR) patterns from community pharmacy dispensing data — 2–4 weeks before cases appear in facility-based reporting systems like DHIMS2. Built on live dispensing data from the KAM AID Pharmacy network (Greater Accra), it surfaces actionable alerts for GHS surveillance officers at district and regional level. --- ## The Problem Ghana's disease surveillance depends on DHIMS2, which is facility-based and retrospective. Outbreak signals appear in aggregate reports only after disease has circulated in communities for two to four weeks. No signal layer currently sits upstream of the clinic door. ## The Approach Community pharmacies are Ghana's first healthcare touchpoint. When malaria surges, patients buy antimalarials before they visit a clinic. When a gastroenteritis cluster starts, ORS and zinc sales spike first. Aduru Sentinel listens to those signals. **Pipeline:** 1. Ingest anonymised dispensing data from a pharmacy network 2. Map drug-class sales to disease proxies using a pharmacist-curated classification table 3. Apply Z-score anomaly detection with Ghana-specific seasonality baselines 4. Surface geospatial alerts to GHS officers with severity tiers and recommended actions --- ## Signals Detected | Disease Proxy | Drug Class Indicator | Alert Threshold | | --- | --- | --- | | Malaria | Antimalarials | Z ≥ 2.0 (HIGH) | | Gastroenteritis / Cholera | ORS & GI | Z ≥ 1.8 (HIGH) | | Acute Respiratory Infection | Respiratory | Z ≥ 2.0 (MEDIUM) | | Febrile illness (broad) | Antipyretics | Z ≥ 2.5 (MEDIUM) | | AMR signal — Watch tier | Watch Antibiotics | Z ≥ 2.0 (HIGH) | | AMR signal — Reserve tier | Reserve Antibiotics | Z ≥ 1.5 (CRITICAL) | AMR alerts follow WHO AWaRe classification (Access / Watch / Reserve). --- ## Dashboard Four tabs for GHS su …
