Yet its age-standardised diabetes mortality rate is **3.2 times higher**, a gap that has widened over 23 years as Canada's rate declined while Nigeria's showed no sustained downward trend. This divergence points to health system performance gaps rather than disease burden alone. An estimated **78.7% of people with diabetes in Nigeria remain undiagnosed** and are therefore largely outside the reach of formal care. Nigeria has made substantial investments in primary health care infrastructure, including the expansion of the NPHCDA network and the deployment of approximately 43,000 Community Health Extension Workers (CHEWs), providing a foundation on which targeted adaptations can be built. ## Central Finding Nigeria's recorded diabetes prevalence is lower than Canada's - 3.0% versus 7.7%.
