Fragmented medical record-keeping and an untracked over-the-counter (OTC) pharmaceutical economy represent two persistent and related barriers to health-system efficiency in emerging markets. This paper examines these barriers through a case study analysis situated within a broader research question: how national identification (ID) infrastructure can be leveraged to achieve health data interoperability in resource-constrained settings. Drawing on secondary market data, regulatory documentation, and a single illustrative case, a pre-seed Egyptian healthtechnology venture piloting a National ID-based interoperability layer, this paper analyzes the structural causes of medical record fragmentation and the scale of Egypt's informal OTC market, variously estimated between US$385 million and US$725.76 million depending on research methodology. The analysis situates Egypt's Universal Health Insurance Law No. 2 of 2018 within a comparative framework alongside India's Aadhaar-linked health identity system and Estonia's X-Road data-exchange platform, identifying transferable design principles and adoption risks. Findings suggest that National ID-based interoperability can meaningfully reduce transaction friction between clinics, pharmacies, and insurers, but that exclusion risk, standardization cost, and regulatory lag remain material constraints. The paper concludes with implications for policymakers, insurers, and health-technology ventures operating in similarly fragmented markets.
