This is the survey tool used in Ethiopia to assess the association between water insecurity, nutrition, and mental health. Data collection for this community-based cross-sectional study was conducted with women residents in rural households in Haramaya woreda (district), East Hararghe Zone, Oromia Region, Ethiopia, from July to September 2019.
