IDI Annual Observatory Report

This report presents the Institute for Decision Infrastructure's annual assessment of institutional decision architecture quality for 2025. It applies the Institute's analytical framework to documented public-record evidence across 14 sectors, 7 global regions, and 27 governance failure cases.

Key findings: Global average decision architecture quality: 3.2/10 (2025). No sector scores above 4.0. Cost-bearer representation is the lowest-performing structural area in every sector, every region, and every year of the evidence base without exception. Documented governance failure costs reviewed: $3.14 trillion (cases 2003–2025).
