This systematic review examines the agronomic and socioeconomic impacts of fall armyworm (Spodoptera frugiperda) on smallholder maize systems in sub-Saharan Africa from 2016 to 2025. Following PRISMA guidelines and a PICOTS framework, 30 eligible studies were synthesized to assess yield losses, infestation levels, livelihood disruptions, and food security outcomes. The evidence indicates substantial maize yield losses (20–50%), high infestation rates, and severe crop destruction during early outbreaks, particularly in rain-fed systems with limited pest management access. Beyond agronomic impacts, FAW has reduced household income, increased production costs and labor demands, and negatively affected food availability. However, socioeconomic and nutrition-related outcomes remain underreported, revealing an important research gap. This review highlights FAW as both an agricultural and livelihood threat and underscores the need for integrated pest management strategies, strengthened extension systems, and coordinated policy responses to protect food and income security among smallholder farmers in Africa.
