Background: Medication non-adherence in schizophrenia remains a critical determinant of relapse, poor functional outcomes, and increased healthcare utilization. Identifying modifiable predictors of adherence is essential for informing targeted and scalable interventions, particularly in resource-constrained settings.  Methods: A hospital-based cross-sectional study was conducted among 187 patients with schizophrenia at Komfo Anokye Teaching Hospital, Ghana. Data were collected using structured questionnaires, including the Medication Adherence Rating Scale (MARS). Associations between adherence and predictor variables were examined using Pearson’s correlation and multivariable linear regression. Statistical significance was set at p < 0.05.
