The study generated an ordinal, school level dataset using a cross sectional, workshop based design to support risk based prioritization of sanitation operation and maintenance processes in Kampala City, Uganda. Data were collected through five calibration workshops involving 205 participants, each representing a single school and comprising head teachers or designated sanitation teachers. Schools were selected using proportionate stratified random sampling from an estimated population of 852 schools, stratified by administrative division, funding type, and education level. Participants completed a structured questionnaire adapted from the World Health Organization Sanitation Safety Planning framework to assess 33 predefined O&M indicators across six domains: service planning, facility design standards, resource provision and management, service delivery, governance, and monitoring and evaluation. Each indicator was rated on three dimensions: status of implementation, likelihood of failure, and severity of consequences. Status and likelihood were scored on five point ordinal scales, while severity was scored using a non linear five level scale (1, 2, 4, 8, 16) to reflect increasing consequence magnitude. The resulting dataset was analysed using non parametric descriptive statistics to derive median and upper quartile risk scores for indicator prioritization.
