This protocol describes the ON-THE-MOVE research study, which examines inter-facility mobility and engagement with antiretroviral therapy (ART) care among people living with HIV in the Western Cape, South Africa. The mixed-methods study combines analysis of routine health data, a realist evaluation, and human-centred co-design to understand how mobility influences engagement and treatment outcomes and to inform interventions and health-system adaptations that support sustained HIV care.
