Beyond the Textbook: Guidelines vs. Clinical Practice: Diabetes Mellitus, From “Control the Sugar” to Protecting the Person is a clinical education compilation focused on the everyday decisions that sit between what diabetes guidelines recommend and what clinicians actually have to manage in practice. Diabetes care is more than lowering blood glucose. The clinician has to establish the diagnosis, recognise urgent problems, determine the type of diabetes, assess complications and comorbidities, choose appropriate treatment, and consider whether the patient can understand, obtain and sustain that treatment. The compilation brings these issues together in a practical clinical narrative. It covers diagnosis and glucose testing, hyperglycaemia and hypoglycaemia, classification of diabetes, comprehensive assessment, lifestyle and pharmacological treatment, insulin use, glycaemic targets, prevention of complications, sick-day management, diabetes in pregnancy, childhood and adolescent diabetes, affordability and access to medicines, and the realities of diabetes care within the Nigerian health system. The emphasis throughout is on translating evidence and guideline recommendations into care that is clinically appropriate and workable in the patient's real circumstances. Nigerian and African evidence is incorporated where it helps illustrate the challenges of access, affordability, continuity and delivery of diabetes care.
