Introduction: Breast pain (mastalgia) is a common reason for radiological consultation and a major source of anxiety due to fear of breast cancer. The aim of this study was to determine the BI-RADS classification of breast ultrasound findings in patients presenting with mastalgia and to assess their correlation with clinico-demographic data. Methods: This was a retrospective, descriptive, and analytical study conducted over a three-year period (January 1, 2019 to December 31, 2021) in the Medical Imaging Department of Mitsinjo Betanimena University Hospital in Toliara. All female patients who underwent breast ultrasound for mastalgia were included. Statistical analysis was performed using SPSS version 25, with a significance level set at p < 0.05. Results: Among 154 breast ultrasounds performed, 54 (35.06%) were indicated for mastalgia. The mean age was 35.5 years, and 68.5% of patients were aged 18–45 years. Ultrasound findings were classified as follows: BI-RADS 1 (31.5%), BI-RADS 2 (55.5%), BI-RADS 3 (5.5%), BI-RADS 4 (7.5%). A significant association was found between BI-RADS classification and pain laterality, as well as the presence of associated clinical signs. Conclusion: Mastalgia is most often associated with benign findings. However, the presence of suspicious lesions highlights the important role of breast ultrasound, particularly in settings where mammography is not readily available.
