Mutualité en santé et organisation communautaire : une approche communicationnelle fondée sur le modèle des quatre flux

Cette thèse analyse la manière dont des communautés rurales sénégalaises s’organisent collectivement pour faire face aux difficultés d’accès aux soins, à travers un mécanisme endogène de financement de la santé : le champ communautaire. Elle s’inscrit dans une approche qualitative, interprétative et ethnographique, fondée sur une immersion prolongée menée entre février et mai 2025 dans deux villages, Keur Momath Mbayang (région de Fatick) et Nguérane Fass (région de Kaffrine). S’appuyant sur le modèle des quatre flux de la communication organisante proposé par McPhee et Zaug (2000), la recherche examine comment la communication ne se limite pas à accompagner l’action collective, mais participe activement à la constitution, à la coordination et à la légitimation du dispositif communautaire. La démarche méthodologique combine vingt-quatre entretiens semi-directifs, cinq groupes de discussion, trois séances d’observation participante et un journal de terrain réflexif, permettant une analyse fine et située des pratiques observées. Les résultats montrent que le champ communautaire dépasse largement sa fonction économique. Il apparaît comme un espace communicationnel central où se construisent les règles, se négocient les formes de participation et se consolident les relations de confiance. L’auto-structuration produit des normes évolutives ancrées dans le contexte local ; la coordination rend possible l’action collective au quotidien ; la négociation de l’adhésion entretient l’engagement moral des membres ; et le positionnement institutionnel inscrit le dispositif dans un environnement social et institutionnel élargi. L’analyse met en évidence que la durabilité de ce mécanisme repose sur l’entrelacement dynamique de ces flux, qui font de la communication une pratique organisante essentielle à la production de la solidarité et à la gouvernance locale du soin. En croisant communication organisationnelle et protection sociale communautaire, cette thèse contribue à une compréhension renouvelée des formes africaines d’organisation collective de la santé, en soulignant le rôle central des pratiques communicationnelles dans la transformation des vulnérabilités sanitaires en capacités collectives. This thesis examines how rural Senegalese communities collectively organize to address challenges in access to healthcare through an endogenous health-financing mechanism: the community field. Adopting a qualitative, interpretive, and ethnographic approach, the study is based on prolonged field immersion conducted between February and May 2025 in two villages, Keur Momath Mbayang (Fatick region) and Nguérane Fass (Kaffrine region). Drawing on the four-flows model of constitutive organizational communication proposed by McPhee and Zaug (2000), the research explores how communication does not merely accompany collective action but actively contributes to the constitution, coordination, and legitimation of the community-based mechanism. The methodological design combines twenty-four semi-structured interviews, five focus groups, three participant-observation sessions, and a reflexive field journal, enabling a fine-grained and situated analysis of social practices. The findings show that the community field extends far beyond its economic function. It emerges as a central communicative space in which rules are constructed, participation is negotiated, and relations of trust are consolidated. Processes of self-structuring generate context-specific and evolving norms; coordination enables day-to-day collective action; negotiation of membership sustains members’ moral commitment; and institutional positioning situates the mechanism within a broader social and institutional environment. The analysis demonstrates that the sustainability of this mechanism depends on the dynamic interweaving of these communicative processes, which make communication an essential organizing practice for the production of solidarity and the local governance of care. By bridging organizational communication and community-based social protection, this thesis contributes to a renewed understanding of African forms of collective health organization, highlighting the central role of communicative practices in transforming health vulnerabilities into collective capacities.
